HOW TO CLAIM A PRIZE: For all Sour and Tart Grand and First Prize packages, mail the entire original winning game wrapper and light-up LICK-O-METER game piece (no copies or mechanical reproductions will be accepted) and a separate 3"x5" card with your name, complete street address, day/evening phone numbers, birth date and location where you purchased your candy and mail to: Eat, Lick & Win! Prize Claim, P.O. Box 1409, Young America, MN 55594-1409. Prize claim must be received by 2/14/06. If you are an eligible minor in the state in which you reside, include your parent or legal guardian's name and address on the 3" x 5" card. You may wish to send your prize claim via a traceable form of delivery, but it is not required; however, all prize submissions are made at your sole risk and proof of mailing does not constitute proof of delivery. For Grand Prize or First Prize winners, it is recommended that you retain a photocopy of the original game piece and wrapper for your records. All prize claims are subject to verification. Following verification of a prize claim, the Grand Prize winner will be notified by a traceable form of delivery. First Prize winners will receive their prize by a traceable form of delivery. Second prize winners will receive their prize by mail. If a prize notification or prize is returned as undeliverable, the prize will be forfeited. One (1) Grand Prize will be awarded in a Second Chance Drawing. ALL OTHER UNCLAIMED PRIZES WILL NOT BE AWARDED.
